On November 2, 2010 the Department of Labor’s Employee Benefits Security Administration and the Department of Health and Human Services released a revised model notice related to the Children’s Health Insurance Program (CHIP). This revised version of the model CHIP notice replaces the earlier version released on September 1, 2010.
The header and explanatory language in the model notice are unchanged, as are the number and identity of the various state Medicaid and CHIP programs currently available. However the pagination has been adjusted permitting users to print reader-friendly copies. Also, the following states have revised either their Internet website addresses or their telephone numbers, which are shown in this updated version:
